Are you passionate about promoting health equity and improving access to healthcare for Indigenous community members? If so, we have a wonderful opportunity for you! The Indigenous Health Navigator will work with clients in the One Window Resource Centre as a support to help individuals navigate through the health care system, maintain relationships, offer assistance, and advocate when necessary.

 

Qualifications:

  • Social work or related experience working within an Indigenous wellness framework and/or community outreach

  • Demonstrated knowledge and understanding of Indigenous cultures, histories, and health disparities

  • Experience in cross-cultural settings

  • Asset-based approach to patient care

  • Demonstrated effective interpersonal skills in a variety of settings

  • Proficient in computer skills

  • Able to work independently

  • Ability to problem-solve and use critical thinking skills

  • Ability to build relationships and rapport with different types of people, including clients, family members, agency support workers, and health care providers

  • Excellent written and spoken communication skills

  • Flexible, responsive, and adaptable to evolving client and health care provider’s needs

  • Valid driver’s license

  • Current, cleared Criminal record check and child intervention check

 

Responsibilities:

  • Establish close relationships with and serve as primary point of contact for clients

  • Home visits on a regular basis

  • Engage with clients and families to understand their specific healthcare needs and preferences

  • Transport clients to appointments as needed

  • Accompany clients to medical appointments, when required

  • Deliver in-home weekly meals as required

  • Establish partnerships with health care providers

  • Advocate for the rights and interests of clients, ensuring they receive respectful and equitable treatment within the healthcare system

  • Identify resources for clients to overcome barriers to care

  • Remain aware of current services offered by service providers, (such as mental health and addictions support available)

  • Maintain documentation and complete all required reporting requirements in a timely manner

  • Maintain accurate and confidential records of client engagement and progress

  • Attend relevant meetings and represent the organization in a professional manner, as requested by the Supervisor
